Prior to your appointment, please pick up a pre-appointment package of forms to complete available at the library or they can be downloaded from this website. Rather than downloading each document separately from the “What To Bring with You” page, you can use this link (coming soon) to download the ENTIRE Wharton pre-appointment package.

Kindly fill out the package documents as best you can BEFORE your appointment. If you cannot pick up or download the documents prior to your appointment, please arrive half an hour before your appointment so the documents can be completed.

COVID-19 NOTICE: Masks are not required but if you are experiencing Covid symptoms, tested positive or come in close contact with someone who tested positive for COVID- 19 in the past 10 days, please call and reschedule your appointment.
